Diabetes Management Center

“Better health through education”

The Diabetes Management Center at CMMC was created to provide individualized education to the patient, their families, and support system about diabetes self-management and related issues. We strive to provide the highest quality, cost efficient diabetes self-management training to positively affect patient outcomes.

DMC provides outpatient diabetes self-management training with a coordinated interdisciplinary approach:

Certified Diabetes Foot Specialist
Exercise Physiologist *
Health Education Specialist *
Registered Dietitian *
Registered Nurse *

* Certified Diabetes Educators by the National Certification Board for Diabetes Educators.

Services:

Inpatient Diabetes Self-Management Training is provided as part of routine patient care:

  • Survival skills are provided including diabetes physiology, medications, glucose monitoring, and nutrition management.

Outpatient Diabetes Self-Management Training

  • Comprehensive diabetes self-management training is provided including diabetes physiology, regulation of blood glucose, blood glucose monitoring, nutrition management, medications, activity / exercise guidelines, short term and long term complications, and the psychology of living with a chronic disease.
  • Outpatient diabetes self-management training is provided in classes of less than 10 persons or in one-on-one consultations.
  • Medicare reimbursable. (Patient should check with personal insurance to verify coverage)
  • Recognized by the American Diabetes Association Education Recognition Program.

Continuous Glucose Monitoring System

  • Physician's referral required
  • Up to 288 blood glucose readings a day – up to 864 readings in a 72-hour time period.
  • This diagnostic service helps get blood glucose control right!
  • Medicare reimbursable. (Patient should check with personal insurance to verify coverage)

Diabetes Support Group

  • Meets third Tuesday of every month 10:00 a.m. – 11:00 a.m.

Diabetes Management Center classroom (3 rd floor CMMC)

  • Past topics included:

    • Super Market Tours (guided tour in a local supermarket to demonstrate label reading and analysis to promote healthy, cost minded food choices)
    • Depression and Chronic Disease (presented by our Psychiatric support staff)
    • Holiday Tasting with cookbook (staff routinely cooks for Support Group)
    • Holiday Sharing with recipe swap (this December activity has become a tradition with Support Group members bringing in a holiday favorite to taste and recipe to share – and no, recipes do not have to be heart healthy on this day!)
    • Community Awareness, Recognition and Treatment of insulin resistance to delay onset of diabetes and prevent the onset of complications.

  • Community health fairs (organized through CMMC Marketing Department )
  • Senior Health programs (organized through CMMC Marketing Department and Diabetes Foundation of Mississippi )
  • School Health programs (organized through CMMC Marketing Department and Diabetes Foundation of Mississippi )
  • Camp Kandu and Not a Sweet Subject(organized through Diabetes Foundation of Mississippi ). Camp Kandu is a diabetes day camp for children with diabetes that has a strong education component for parents. While the children play under the direction of volunteer expert diabetes trained staff, the parents learn about the day-to-day living issues of having a child with diabetes. Not A Sweet Subject is an education program for school and / or daycare staff wanting to know more about safety, management, teaching and working with children with diabetes.
